Medical Scheduler for Homecare Agency, FT, Benefitted

The Hospice Patient Services Specialist/Scheduler is responsible for the orderly scheduling of Hospice patient visits and staff scheduling. Receives and triages telephone calls related to patient care, scheduling and any other issues that may arise. Notifies staff and the appropriate managers of changes in patient scheduling. The Hospice Patient Services Specialist/Scheduler should be knowledgeable in all phases of Agency policy and services. The Hospice Patient Services Specialist/Scheduler reports to the Hospice Director. **EDUCATION AND EXPERIENCE** 1\. Experience in a medical office practice or similar setting essential; excellent public relations skills required. 2\. Scheduling experienced preferred. **QUALIFICATIONS** 1\. Demonstrated ability in office procedures including computer skills. 2\. Sound organizational skills. Ability to coordinate and manage multiple activities simultaneously. 3\. Ability to handle telephone communication in a manner that preserves good public relations. 4\. Dependable. Capable of using initiative and good judgment. **JOB FUNCTIONS:** 1\. Schedules hospice care services under supervision, accurately, completely and in accordance with established Agency procedures and standards. 2\. Assigns patient cases to the appropriate case manager. 3\. Completes assigned daily workflows per HCHB protocols. 4\. Successfully completes OLH education requirements as assigned in established timeframes. 5\. Notifies appropriate individuals of any change or concerns related to patient schedule or care. 6\. Keeps Hospice Manager and Hospice Director informed of staffing needs. 7\. Receives and triages telephone calls. Responds to and assists clinicians with insuring that their schedule is accurate. 8\. Establish vacations/time off on scheduling calendar – notifying supervisor and CM prior to time off to reschedule and/or coordinate reassignment of needed visits. 9\. Alert staff of unscheduled visits via DM each morning by 10am. Accept phone calls from and works with clinical staff in reassignments when visits are cancelled to ensure full productivity. 10\. Schedule patient Recertification visits or Discharge visits to ensure orders are appropriate for 60\-day episode and/or Discharge. 11\.
