**Overview**
Emerald Wings Hospice is a locally owned, women\-led hospice agency based in St. Paul, Minnesota. Our team is dedicated to providing compassionate, patient\-centered end\-of\-life care that supports not only the patient, but the entire family.
We are seeking a compassionate **Medical Social Worker (MSW)** to support patients and families navigating end\-of\-life care. As part of our interdisciplinary hospice team, the social worker plays a vital role in addressing the emotional, psychosocial, and practical needs of patients and their loved ones.
In this role, you will provide guidance, counseling, and resource coordination to help families through one of life’s most meaningful transitions. This position offers the opportunity to make a profound impact by ensuring patients and families feel supported, heard, and cared for throughout their hospice journey.
**Responsibilities**
* Conduct psychosocial assessments to evaluate patient and family needs related to illness, caregiving, and end\-of\-life planning
* Provide emotional support and counseling to patients and family members coping with serious illness and grief
* Assist families in understanding hospice services and navigating available community resources
* Collaborate with the interdisciplinary hospice team (nurses, chaplains, physicians, and aides) to develop patient care plans
* Support patients and families with advance care planning and end\-of\-life decision making
* Facilitate communication between patients, families, and the care team
* Document visits and assessments according to hospice care standards and regulatory guidelines
* Provide bereavement support and guidance to families as appropriate
**Requirements**
* Master’s Degree in Social Work (MSW) from an accredited program
* Current Minnesota social work license (LSW, LGSW, LICSW, or equivalent)
* Valid driver’s license and reliable transportation for patient visits
* Strong interpersonal and communication skills with the ability to support families during sensitive and emotional situations
* Ability to work collaboratively within an interdisciplinary healthcare team
Preferred
* Experience in hospice, palliative care, healthcare, or medical social work
Pay: $30\.00 \- $42\.00 per hour
