Are you a compassionate and dedicated social worker looking to make a real difference in the lives of patients and families facing end\-of\-life challenges? At Mountain Valley Hospice, we provide exceptional care and support, and we’re seeking a **PRN** **Hospice Social Worker** to join our team. This role is more than a job—it’s an opportunity to bring comfort, guidance, and advocacy to those who need it most.
### **What You’ll Do:**
✔ **Support \& Advocate** – Assess patients and families, develop personalized care plans, and provide emotional and practical support to enhance coping skills.
✔ **Educate \& Guide** – Help families navigate the dying process, anticipatory grief, and bereavement while ensuring they have the resources they need.
✔ **Collaborate \& Communicate** – Work closely with an interdisciplinary team, including nurses, chaplains, and volunteers, to advocate for patient needs.
✔ **Connect to Resources** – Make referrals to community organizations, chaplains, and volunteer programs to provide additional support.
✔ **Stay Organized** – Maintain timely and accurate documentation to ensure seamless care for each patient.
### **What We’re Looking For:**
✔ A **bachelor’s or master’s degree** in social work, sociology, or psychology from an accredited institution.
✔ At least **one year of social work experience** in a healthcare setting.
✔ A compassionate professional who is comfortable traveling to **patients’ homes, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and hospitals** to provide care.
