Hospice Social Worker

Homeside Hospice, a nurse owned and operated company is seeking a Full Time Hospice Social Worker to work with 3 other passionate social workers with 20 plus years of experience**. Must be willing to travel to multiple Counties in New Jersey (Union, Somerset, Middlesex, Mercer, Monmouth, Upper Ocean**). Collegial atmosphere, with supportive and proactive team, that embraces social work core values, advocacy and the goal of providing the highest level of service provision. On\-call responsibilities (including weekends) are on a rotation of once every 5 weeks. Job duties include but are not limited to; * Responsible for the delivery of varied social work services to adult (only) hospice patients and their families * Provide comprehensive initial psycho\-social and bereavement assessments, reassessments * Direct casework/social work services, * Individual and family counseling as needed, crisis intervention, community referrals * Engages in advocacy role * Provides bereavement services Education: Licensed by the State Board of Social Work Examiners, with a Master’s degree in social work from a graduate school of social work accredited by the Council on Social Work Education, and at least one year of post\-master’s social work experience in a health care setting in accordance with N.J.S.A. 45:15BB\-1 et seq and N.J.A.C. 13:44G. Motivated to train the right candidate. Must have current LSW, or LCSW.
