As a Hospice Social Worker, you will play a vital role in our journey to provide personalized support and peace for patients and families in Douglas. We are seeking a highly skilled and empathetic social worker to join our dedicated team. **Responsibilities:** * Provide emotional, psychological, and social support to patients and their families, assessing their needs and developing individualized care plans to meet those needs * Collaborate with interdisciplinary teams, including physicians, nurses, chaplains, and other healthcare professionals to ensure comprehensive care * Develop and maintain relationships with patients, families, and caregivers, addressing social, emotional, and spiritual concerns * Identify and address social determinants of health, such as housing, transportation, and financial needs, to improve patient outcomes * Facilitate communication between patients, families, and healthcare providers to ensure informed decision\-making * Provide education and support to patients and families on end\-of\-life care, coping mechanisms, and bereavement * Document all interactions and assessments in accordance with agency policies and regulations **Requirements:** * Master's degree in Social Work (MSW) * Active, unrestricted Wyoming LSW or LCSW license * Strong understanding of social work theories, practices, and ethical standards * Excellent communication, interpersonal, and counseling skills * Ability to work independently and as part of a team * Strong problem\-solving and organizational skills * Comfort working with diverse populations and cultures **What We Offer:** * Flexible scheduling **How to Apply:** If you are passionate about delivering compassionate hospice care and are looking for a dynamic and supportive work environment, we encourage you to apply for this rewarding opportunity.
