**Job Overview**
We are seeking a compassionate and highly skilled Hospice Registered Nurse to provide exceptional end\-of\-life care to patients in their homes.

**Contract Length: 13\-week** contract
**Pay Rate: Range $2700 to $2800 Weekly Gross pay** based on a 40\-hour work week (Includes non\-taxable travel/housing and M\&I stipend available for qualified/eligible travelers)
**Mileage Reimbursement:** $0\.49/mile for distance covered between patient home appointments
**Unit:** Home Health Hospice
**Shifts available: Days: 8 hours or 10 Hours**
**Shift timings: Days, 8\.00 a.m. – 4\.30 p.m or 9\.00 am \- 7\.30 pm**
**Schedule:** 40 hours/week guaranteed, 5 X 8\-hr shifts per week with one weekend rotation per month (or) 4 X 10\-hr shifts per week (any 4 days of the week \- flexible days, posted 4 weeks in advance)
**Position Requirements:**
* Registered Nurse: 1 year \- Required
* Recent/Current Hospice nursing experience in a Home Health setting: 1 year within the last 3 years – Required
* Home Care Home Base (HCHB) charting \- Required
* Must have valid Driver’s License and Personal Auto Insurance
**Education, License and Certification Requirements:**
* ASN or an equivalent – Required
* BSN or an equivalent – Highly preferred
* RN License for State of the state applying or Compact if acceptable \- Required
* BLS Certification from AHA or American Red Cross \- Required
**Job Description:**
* Provide and coordinate care and case management for assigned hospice patients in comfort of their homes including admissions
* Develop and implement nursing care plans for visits as assigned.
* Performs Case Management and all types of visits for the assigned patient case load
* Support patient comfort, pain management and ability to stay at home \- enabling them to spend quality time with their loved ones.
* Empower patients and families by educating them on disease management and pain management.
* Provide bereavement support.
* Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
* Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
* Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
* Notify physicians of significant changes in status.
* Document and complete charting for all patient visits within 24 hours.
* Provide bereavement support to patient’s families as needed.
* Territories to cover: Seattle/Shoreline (For 4 X 10\-hr shifts positions)
* Territories to Cover: North Bend/Carnation/Bellevue/Redmond areas (For 5 X 8\-hr positions)
Pay: $2,700\.00 \- $2,800\.00 per week
