**Job Summary** We are seeking a compassionate and dynamic Hospice Chaplain/Spiritual Care Coordinator to provide holistic spiritual support to patients, families, and staff within our hospice care program. This vital role offers an opportunity to deliver meaningful spiritual care, foster emotional well\-being, and guide individuals through life’s most challenging moments with empathy and professionalism. The ideal candidate will serve as a spiritual resource, advocate for diverse faith backgrounds, and promote comfort and peace during end\-of\-life journeys. This paid position is designed for someone passionate about making a positive impact through spiritual guidance, crisis intervention, and education. ***Willing to travel all of Contra Costa: Martinez, Clayton, Walnut Creek, Pleasant Hill, Concord, Danville, San Ramon, Moraga, Lafayette, Alamo, Orinda, Hercules, Richmond, El Sobrante, Pinole, San Pablo, Bay Point, Pittsburg, Antioch, Brentwood, Discovery Bay, Bethel Island, etc,.*** **\*Rates are determined by experience. Rates are per visit, not per hour\*** **Duties** * Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. * Facilitates the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs. * Provides direct spiritual care to patients and families/caregivers. * Serves as liaison and support to community chaplains and spiritual counselors. * Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. * Documents direct services and ongoing communication with community chaplains and spiritual counselors. * Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. * Provides consultation, education, and support to the interdisciplinary group on spiritual care. * Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care. * Provides for funeral or memorial services for patients as requested. * Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. * Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers. * Actively participates in quality assessment performance improvement teams and activities. * Accepts and performs other related duties and responsibilities as required. **Qualifications** * Bachelor’s degree in Theology, Pastoral Care, Counseling, or related field; Master’s degree preferred * Certification in hospice chaplaincy or equivalent credentialing highly desirable * Proven experience in hospice care or spiritual counseling with a strong background in crisis management and bereavement support * Excellent communication skills—both written and verbal—with the ability to engage diverse audiences effectively * Strong presentation skills for leading workshops, seminars, and community outreach efforts * Demonstrated leadership abilities in mentoring staff or volunteers within a healthcare or community setting * Skilled in conflict management, crisis intervention, Christian counseling (or other faith\-based counseling), and event planning * Compassionate listener with exceptional empathy skills capable of providing comfort during difficult times * Ability to handle sensitive situations with professionalism while maintaining confidentiality Join us in making a profound difference by guiding individuals through their final journeys with dignity, compassion, and unwavering support.
