Working within a multidisciplinary hospice team, the Chaplain offers guidance, comfort, and hope tailored to the individual’s beliefs and needs, helping them find meaning and peace during this difficult time. **Responsibilities** -------------------- + Provide spiritual care and counseling to patients and their families coping with end\-of\-life issues, grief, and loss + Work collaboratively with the hospice team, including RNs, LPNs, CNAs, social workers, volunteers, and other professionals to support the patient’s holistic plan of care + Assess patients’ and families’ spiritual and emotional needs and incorporate these into the care plan + Facilitate rituals, prayer, or other spiritual practices as requested to support patients’ and families’ beliefs + Serve as an advocate for patients’ spiritual and religious rights and preferences + Offer bereavement support and grief counseling to families before and after the patient’s death + Provide crisis intervention and support during times of spiritual distress + Document spiritual assessments, interventions, and patient/family responses in medical records**Requirements** ---------------- + Ordained clergy or equivalent spiritual counselor credential with experience in hospice or palliative care preferred + Strong interpersonal and communication skills with the ability to support diverse religious and spiritual beliefs + Ability to work effectively as part of a multidisciplinary team + Empathy, compassion, and the ability to provide comfort in challenging emotional situations + Basic computer skills for documentation**About the Company** --------------------- Interim HealthCare \- New Boston, OH is dedicated to providing exceptional home health and hospice care to our community.
