Hospice Chaplain and Bereavement Coordinator

**Overview** White Oak Hospice is a locally owned, family\-operated hospice focused on compassionate, patient\-centered care. **Position Summary** Provides spiritual care and bereavement support to hospice patients, families, and caregivers, while collaborating with the interdisciplinary team. **Key Responsibilities** * Conduct spiritual assessments and provide direct patient/family support * Participate in care planning and team meetings * Coordinate with community chaplains and spiritual counselors * Provide grief counseling (individual and group) for up to 1 year post\-loss * Support hospice staff with grief resources * Plan and assist with memorial services * Maintain accurate documentation Benefits: * Dental insurance * Health insurance * Life insurance * Mileage reimbursement * Paid time off * Retirement plan * Vision insurance Work Location: In person
