Hospice Care, Bereavement Counselor (LCSW or LMSW)

Job Description Provides counseling services to bereaved family members of hospice patients and the general community. Job Responsibility 1\.Assesses bereaved clients and provides program orientation. 2\.Collaborates with healthcare team members to develop and implement client plan of care. 3\.Conducts individual, group and family counseling sessions. 4\.Serves as a liaison to referral sources to facilitate continuity of care. 5\.Performs related duties as required. Job Qualification * Master's Degree required. * Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State, or Current License to practice as a Mental Health Counselor in New York State required, plus specialized certifications as needed. * 1\-3 years of technical experience required. * Minimum of 1 year of bereavement and grief counseling experience strongly preferred. * Experience with individual and group sessions and assessment, experience working with all ages, and families preferred. * Hospice care, home care, or community health experience preferred. * **Additional Salary Detail** The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
