We are currently seeking a compassionate and motivated Bereavement Coordinator to join our growing hospice team. This role is ideal for a social worker who is passionate about grief support, family care, and the human side of healthcare.
The Bereavement Coordinator plays a vital role in ensuring families receive meaningful support after the loss of a loved one. Through follow\-up, counseling, support resources, and memorial events, you will help guide families through the grieving process for up to 13 months following the patient’s passing.
**Position Overview**
The Bereavement Coordinator is responsible for developing, coordinating, and maintaining Everlight Hospice’s bereavement program.
In this role, you will provide grief support, coordinate memorial events, and collaborate with the interdisciplinary team to ensure families receive compassionate and individualized bereavement care.
This position offers the opportunity to build relationships with families, provide meaningful emotional support, and help shape a growing hospice program committed to compassionate care.
Bereavement services may include:
* Follow\-up calls and check\-ins with families
* Grief support groups
* Individual grief counseling when appropriate
* Educational resources and mailings
* Referrals to community grief and mental health resources
* Coordination of memorial services and remembrance events
**Key Responsibilities**
* Develop, coordinate, and maintain Everlight Hospice’s bereavement program.
* Conduct bereavement risk assessments and assist the interdisciplinary team in developing individualized bereavement care plans.
* Provide grief support services including follow\-up calls, support groups, educational resources, and individual counseling when appropriate.
* Identify individuals at higher risk for complicated grief and coordinate additional support or referrals.
* Coordinate memorial services and remembrance events for hospice families.
* Collaborate with nurses, physicians, social workers, chaplains, and volunteers as part of the interdisciplinary hospice team.
* Assist with recruitment, training, and coordination of volunteers who support bereavement services.
* Maintain accurate documentation of bereavement services in accordance with hospice policies and regulatory requirements.
* Develop and maintain a directory of community grief resources.
* Provide support and resources to hospice staff coping with cumulative grief associated with hospice care.
**Qualifications**
**Required**
Bachelor’s or Master’s degree in Social Work.
We welcome candidates with the following credentials:
* BSW
* MSW
* CSWA
* LCSW
**Preferred**
* Experience in hospice, palliative care, counseling, social work, chaplaincy, or behavioral health.
* Training or education related to grief and loss counseling.
* Knowledge of grief processes and bereavement support.
* Strong interpersonal and communication skills.
* Ability to work independently while collaborating with an interdisciplinary team.
* Strong organizational and documentation skills.
* A compassionate and supportive presence with individuals and families experiencing loss.
**Work Environment**
This role may include office, remote, and community\-based work supporting hospice families. Occasional evening or weekend availability may be needed for memorial services, support groups, or bereavement events.
