**Job Overview**
We are seeking a compassionate and highly skilled Hospice Licensed Master Social Worker (LMSW) to join our dedicated healthcare team. The LMSW will provide comprehensive psychosocial support, counseling, and case management services to patients and their families facing end\-of\-life care. This role involves collaborating with interdisciplinary teams to deliver patient\-centered care in hospice settings, including inpatient, outpatient, home, and community environments. The ideal candidate will possess extensive experience in behavioral health, mental health counseling, and social work within hospice or related healthcare contexts.
**Duties**
* Conduct thorough patient assessments to determine psychosocial needs and develop individualized care plans.
* Provide clinical counseling services including individual, family, and group therapy tailored to hospice patients.
* Facilitate crisis intervention and manage complex emotional responses related to terminal illness and grief.
* Assist with intake procedures, ensuring compliance with HIPAA regulations and proper medical documentation.
* Collaborate with medical teams to coordinate care, including behavioral health interventions such as cognitive behavioral therapy, motivational interviewing, and addiction counseling.
* Manage casework involving patients with diverse conditions such as dementia, Alzheimer’s disease, PTSD, autism spectrum disorders, developmental disabilities, and geriatrics.
* Support families through grief counseling and bereavement services while addressing emotional and psychological challenges.
* Document all clinical interactions accurately within EMR systems and maintain detailed medical records following regulatory standards.
**Skills**
* Proven experience in hospice care or end\-of\-life settings with knowledge of post\-acute care practices.
* Strong background in child \& family counseling, behavioral therapy, assisted living support, senior care, and working with people with disabilities or developmental challenges.
* Expertise in crisis management, mental health counseling, diagnostic evaluation, case management, and social work principles.
* Excellent communication skills for effective patient assessment and clinical counseling across diverse populations including pediatrics and geriatrics.
* Ability to work collaboratively within interdisciplinary teams while maintaining confidentiality per HIPAA guidelines.
* Experience working with inpatient facilities such as nursing homes or outpatient clinics is preferred.
* Knowledge of specialized areas such as Alzheimer’s care, PTSD care, dementia care, working with people with autism or developmental disabilities is advantageous.
