Job Summary: Under the direction of the Home Health / Hospice Director, the clinical manager is responsible for oversight, planning and directing the daily operation of a clinical team. The Clinical Manager ensures compliance with established professional and accreditation standards, practices and regulatory requirements. This position is responsible for the support, management and development of direct report staff, ensuring positive team building, effective and productive care management and meeting relevant organizational strategic goals. Duties \& Responsibilities: **ESSENTIAL DUTIES AND RESPONSIBILITIES** The requirements listed below are representative of the knowledge, skill and/or ability required. Other duties may be assigned. **Clinical Leadership** * Demonstrates clinical leadership with team members * Remains current in clinical interventions * Understands and uses resources of clinical specialists * Able to access, understand and use reports to help manage day to day operations and track activity * Work with scheduling to coordinate the scheduling of staff visits with patients to monitor continuity and appropriateness of visits * Receive voice mail from staff on patient reports and scheduling changes * Investigate and address patient and physician phone calls. Keep case managers updated on all\-important patient information. * Write/lock verbal orders as necessary * Ensure all Medicare COP’s are met \- Monitor orders, 485, authorizations, care etc * Works with Staff Scheduler to insure appropriate coverage of all shifts + Approves Vacation * Be responsible for continuous review of patients on caseload; * Facilitates and Schedules: + POD Meetings + Team Meetings + Case Conferences + AD HOC Safety Huddles * Participates in SAFETY HUDDLE \& Completes Follow up Timely * Works with intake to maintain maximum capacity and meet budget targets * Problem solves clinical issues – seeks advice after initial attempts * Proactively collaborates with education, intake, quality **Quality** * Ensures compliance with all accreditation and regulatory standards for team * Works with quality manager/staff to implement process to improve patient satisfaction * Regularly communicates patient satisfaction feedback information to staff – positive feedback and deals with patient complaints effectively * Works to improve HH compare measures by guiding staff in patient care , communication handoffs and clinical skills * Serves as the point person for staff documentation issues (coordinates all outstanding work) * Coordinate documentation issues with Manager * Reviews staff documentation in the record concurrently * Maintains a balance with staff productivity targets and quality care **Development of staff** * Interviews and hires new staff * Creates and implements mentoring plans for staff to ensure onboarding and professional development * Conducts case conferences with staff * Provides input to education on in\-service needs for staff * Creates and implements development plans for staff * Attends and actively participates in standing committee meetings – TJC, VNA Managers, Clinical Managers, QIT, QAPI, Safety Event Review, Rehospitalization **Finance** * Ensures productivity target is achieved for team members * Creates plans for staff to effectively deal with productivity issues * Works with staff and clinical specialists to manages patient supply costs * Monitors utilization of resources – meets utilization targets for pts on team * Ensures capacity is maintained * Problem solves coverage options * Complete billing audit/monthly reconciliation as needed * Reviews monthly dashboard with Director and implements corrective action plan to address issues not meeting budget target **Performance Management** * Completes staff evaluations in a timely manner * Complete scheduled home visits with all staff on team in timely manner * Demonstrates managerial courage to address performance issues * Effectively addresses staff performance issues + gathers data \& completes analysis + councils staff following HR and union guidelines + Documents performance issues **Personal and Interpersonal skills** * Demonstrates ability to listen, synthesize and communicate information in a clear and concise manner * Utilizes AIDET and A\+ Service Recovery * Commutates and keep informed the following: + Staff + Peers + MD + Management **Leadership and Program Management** * Able to plan and implement changes in practices, policies and ensure institutionalization of change * Able to problem solve and redirect if change process needs adjustment * Able to manage the ‘noise’ of change in a positive way * Is creative in solving problems * Is organized in day to day activities * Takes initiative to identify opportunities for improvement * Leads improvement efforts **Hospice Program Manager – in addition to above** * Responsible for all related Hospice program activities and team members * Activities (bereavement, community events etc) * Team members – SW, Chaplain, Medical Director, HHA, SN etc **OCCASIONAL DUTIES:** Participates in organizational meetings and training venues as requested. Serves on appropriate agency committees. Serves as a preceptor or peer guide for students or new employees as requested. Requirements: **EDUCATION**: Bachelor’s Degree in Nursing or related health field is highly desired. Master’s Degree and ANA Certification preferred. Continuing education courses in nursing preferred. **EXPERIENCE**: A minimum of three (3\) years progressive nursing experience required. Previous experience in a health care setting preferred. Previous supervisory experience required. Home health care environment required. **LANGUAGE SKILLS**: Ability to read and interpret documents such as medical records, medical reports, etc. Ability to write reports, business correspondence and procedure manuals. Must be able to speak, read, and write English with excellent oral and written communication skills required. **REASONING ABILITY**: Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form. Must be able to deal with several abstract and concrete variables. **CERTIFICATES, LICENSES AND REGISTRATIONS** Valid CPR certification required. Valid Rhode Island RN Board license , valid driver’s license and automobile liability insurance is required.
