Job Description
The Chaplain is an integral part of the interdisciplinary care team and as such is responsible for assessing the spiritual needs of the hospice patient and family. Understanding that spiritual services must be delivered in a manner that is consistent with patient/family beliefs and desire for service, the Chaplain coordinates spiritual services offered through the hospice staff, local religious organizations, or community clergy. The Chaplain also serves on the Interdisciplinary Team.
The chaplain reports to the Hospice Director, works with the entire Interdisciplinary team and liaisons with community religious organizations to bring greater understanding of hospice philosophy.
Duties
1\. Participates in evaluating referrals to assure appropriate admissions.
2\. Assesses patient and family for spiritual needs and concerns.
3\. Participates in developing IDT Plan of Care.
4\. Coordinates spiritual care to meet needs and concerns of the patient and family in a manner that is meaningful to them.
5\. Acts as liaison where needed between hospice team and patients/family’s religious organization.
6\. Visits hospice families as directed by the IDT.
7\. Documents all visits and phone calls and turns in documentation in a timely manner.
8\. Attends all IDT team meetings.
9\. Serves as spiritual resource to staff and volunteers.
10\. Responds to patients/families needs after hours when needed
11\. Conducts and coordinates baptisms, weddings, funerals and other religious rites as requested by the patient and family.
Qualifications
1\. Trained in spiritual care by an accepted religious organization.
2\. Hospice training class required
3\. Ordained or credentials in religious organization of choice.
4\. Knowledge and sound understanding of hospice philosophy and it’s concept of care.
5\. Knowledge and ability to apply spiritual counseling techniques in a wide variety of hospice situations.
6\. Ability to work comfortably in a non\-denominational or multi\-denominational environment.
7\. Ability to provide spiritual truths in a comforting manner.
8\. Ability to conduct educational seminars and training for civic, church, volunteer and professional groups.
9\. Knowledge of the process of grief and it’s implications for spiritual care.
Job Type: Full\-time
Pay: $55,000\.00 \- $70,000\.00 per year
