Mission Hospice \& Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end\-of\-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life\-threatening diagnosis through death and bereavement. **This position offers competitive pay, and a supportive work environment.** We are seeking a per diem **After\-Hours Nurse** to provide skilled nursing and hospice care to maximize the comfort and health of patients. **Schedule:** Saturday \& Sunday; 4:30pm \- 12:30am **Office Location:** San Mateo, CA **Coverage Area:** San Mateo, Alameda \& Contra Costa Counties ***Mission Hospice \& Home Care follows all CDPH vaccine requirements for healthcare personnel.*** **Essential Duties \& Responsibilities include** (but are not limited to): * Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. * Under the guidance of the After\-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. * Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family. * Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues. **Education/Experience****:** * One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing. * Experience in acute care/ home health and/or hospice setting preferred**.** **Certificates, Licenses, Registrations:** * Current license as a registered professional nurse in the State of California. * Public Health Nursing certification highly desirable. * HPNA certification preferred. **Pay Range:** $52\.90 \- $66\.20 hourly ***\+ $2\.00 weekend differential*** Mission Hospice and Home Care is proud to serve our community as an equal employment opportunity employer.
